Effective date: October 9, 2026
THIS NOTICE DESCRIBES HOW MEDICAL INFORMATION ABOUT YOU MAY BE USED AND DISCLOSED AND HOW YOU CAN GET ACCESS TO THIS INFORMATION. PLEASE REVIEW IT CAREFULLY.
This notice applies to Lititz Apothecary. If you have questions about it, please contact our Privacy Officer at (717) 626-2222 or info@lititzrx.com. A paper copy is available at the pharmacy at any time.
We use your information to fill your prescriptions and provide pharmacy care, including compounding, vaccines, medication packaging and counseling. We may share it with doctors, nurses and other providers involved in your care. For example, we may call your prescriber to confirm a dose or check for a possible drug interaction.
We use and share your information to bill and get paid by health plans and others. For example, we send your prescription information to your insurance plan so it will pay for your medication.
We use and share your information to run our pharmacy, improve your care and contact you when needed. For example, we review prescription records to check the quality of our work and to train our staff.
We may contact you with refill reminders, notices that your prescription is ready, vaccine appointment reminders, and information about treatment options or health services that may interest you.
Unless you tell us not to, we may share information with a family member, friend or caregiver who is involved in your care or helps pay for it. For example, we may let someone you send pick up your prescription. If you are not able to tell us your preference, for example if you are unconscious, we may share information if we believe it is in your best interest.
We may share information with companies that perform services for us, such as billing and software providers. They must agree in writing to protect your information.
We are allowed or required to share your information in other ways, usually in ways that contribute to the public good, such as public health and research. The law sets conditions we must meet before we can share your information for these purposes.
We will never use or share your information for these purposes unless you give us written permission:
Any other use or sharing not described in this notice also requires your written permission. You may revoke your permission in writing at any time, except to the extent we have already acted on it.
Some types of information have extra protection under Pennsylvania or federal law, such as HIV-related information, mental health information, and drug and alcohol treatment records. When those laws give you more protection than HIPAA, we follow them.
If we receive records about you from a substance use disorder treatment program that are protected by federal rules at 42 CFR Part 2, we will use and share those records only as your written consent and those rules allow. We will not use or share those records, or testimony about them, in any civil, criminal, administrative or legislative proceeding against you unless you give written consent or a court orders it after you have been given notice and a chance to be heard.
When it comes to your health information, you have these rights. To use any of them, contact our Privacy Officer.
You can ask to see or get a paper or electronic copy of the prescription and health records we keep about you. We will usually provide a copy or a summary within 30 days of your request. We may charge a reasonable, cost-based fee.
You can ask us to correct health information about you that you think is incorrect or incomplete. We may say no, but we will tell you why in writing within 60 days.
You can ask us to contact you in a specific way (for example, on your cell phone instead of your home phone) or to send mail to a different address. We will say yes to all reasonable requests.
You can ask us not to use or share certain health information for treatment, payment or our operations. We are not required to agree, and we may say no if it would affect your care. If you pay for a service or item out of pocket in full, you can ask us not to share that information with your health insurer for payment or our operations. We will say yes unless a law requires us to share it.
You can ask for a list (accounting) of the times we have shared your health information for six years before the date you ask, who we shared it with, and why. The list will include all disclosures except those for treatment, payment and health care operations, and certain others (such as any you asked us to make). We will provide one list a year for free, but we may charge a reasonable, cost-based fee if you ask for another within 12 months.
You can ask for a paper copy of this notice at any time, even if you have agreed to receive it electronically. We will give you a paper copy promptly.
If you have given someone medical power of attorney, or if someone is your legal guardian, that person can exercise your rights and make choices about your health information. We will make sure the person has this authority before we take any action.
You can complain to us by contacting our Privacy Officer using the information below. You can also file a complaint with the U.S. Department of Health and Human Services Office for Civil Rights by writing to 200 Independence Avenue, S.W., Washington, D.C. 20201, calling 1-877-696-6775, or visiting www.hhs.gov/hipaa/filing-a-complaint. We will not retaliate against you for filing a complaint.
We can change the terms of this notice, and the changes will apply to all the information we have about you. The new notice will be available on request, in our pharmacy and on our website.
